Member Monthly Data Collections

Below is a sample of the Member Data Collection.  Each member must complete one Data Collection per month of service.

The Data Collection is conducted through Survey Monkey.  A link will be emailed individually each month to the email address one provides at orientation.  This link is uniquely tied to each month's survey and your email address. Please do not forward this message.

This is intended to be a reporting tool and not one's primary data collection tool. Please use a personal tracking system that you prefer.

One recommended method is to copy and paste your data as you enter it into each screen within the survey into a Word document for your records. This Word document will become you and your mentor's reference for past month's data.

Below is a schedule of when Data Collections are due.

Your reporting is very important and allows our program to demonstrate how it is impacting the community.

I. Youth in Action AmeriCorps-SAMPLE- 2011 Data Collection Form

1. What is the primary focus area of your Project?

2. What identified community need are you addressing? For example, sustainability, animal/human rights, poverty, art appreciation, cultural diversity, forest health, reading skills, environmental impact, early childhood development, etc.

3. How are you measuring success or progress in your Project/ What assessment tools are you using?

4. Provide a narrative of how your project is progressing. Be as specific as possible.

5. Project progress/update: How your project is progressing? Include all numbers that you have tracked.

How many community members impacted?  
How many acres?  
How many families?  
How many groups?  

III. Tutoring In Reading Post Test Assessment (Only for members who are tutoring in reading)

1. How many students did you tutor in reading this month?
(Once a person has been counted on this measure, do not count them again for the rest of the year.)

2. How many students who indicated they have increased confidence in reading?
(Once a person has been counted on this measure, do not count them again for the rest of the year.)

3. How many students have increased grade level reading skills by half a grade level?
(Once a person has been counted on this measure, do not count them again for the rest of the year.)

IV. Community Strengthening

1. How many volunteers did you work with this period?
(Once you count a volunteer do not count them again in another data collection.)

2. How many hours did the above volunteers serve? (This is a COMBINED total for all volunteers for this month)

3. Provide a narrative of the activities that volunteers worked on:

4. How many volunteers indicated that they intend to volunteer again?

5. Have (non-AmeriCorps) volunteers increased the capacity to serve the community at your site?

6. How many agencies/groups did you collaborate with this period?

7. List the agencies/groups that you collaborated with.
(Once you have counted an agency do not count again in another data collection.)

V.  Member Development
