When a YiA AmeriCorps member is injured at the mentor site, while during service, they are covered under NAU’s Workers Compensation.
The injury needs to be reported within 24 hours to the:
Early Reporting Claims Service at 1-800-837-8583
Once injury is reported (within 24 hours) the reporting form (download below) needs to be needs to be completed by the member or site and faxed to our office

The original form needs mailed to the office with the member’s signatures.
