Quarterly Reports

One quarterly report is due for each member according to the established schedule below. Your reporting provides valuable feedback for our members and allows our AmeriCorps program to demonstrate how it is impacting the community.

Quarterly Report Instructions

To complete your quarterly report for each member, click on the link below to download the form.

Will be sent through Survey Monkey. Each site and each member will have unique log in access.

First, complete the identifying information such as your name, site, quarter, date, etc.

Section # 1: Needs and Services

Please make sure to provide a narrative on question 4.

Section # 2: Member Development and Evaluation

Questions 1-3 should be straight forward.
Keep us informed of any challenges you have with your member(s) on question 4.
PLEASE recognize your member(s) for outstanding service on question 6.
On question 7, we are looking for honest appraisals of your members progress over the entire year. Therefore, please refrain from giving all top scores in the first quarter report.
As question 8 states, please be sure to discuss your evaluation with you member(s).

Section # 3: Other

Be sure to attach in the email with your quarterly report, any related documents displaying your members service.

Once you have completed your report(s), please attach and email them to submit them through the Survey Monkey system. If you have any questions while completing the form, call 523-2199, for assistance.
