Auto Tuition Payment Plan Enrollment Fee Petition Form

You may petition your auto tuition payment plan enrollment fee through Student and Departmental Account Services. The petition form follows. Complete all sections of the form, and if necessary, attach documentation for your request. You may submit the completed form:

• by fax to (928) 523-1126

Notification of the action taken on the petition is sent to the student's e-mail. (Not for late registration fees or accounts receivable late fees.)
