Tuition Late Payment Fee Petition Form

You may petition your late payment fees through the Office Of The Bursar. The petition form follows. Complete all sections of the form, and if necessary, provide documentation for your request. You may submit the completed form:

  • in person to The Office Of The Bursar, Gammage Building Lobby, Room 124
  • by fax to (928) 523-1126

Normally, notification of the action taken on the petition is done by e-mail, although phone or U.S. Mail are other options available if no e-mail account exists.

(Not for late registration fees or accounts receivable late fees.)

Late Payment Fees are not eligible for appeal once the relevant semester has concluded.
