Tuition Late Payment Fee Petition Form

You may petition your late payment fees through Student and Departmental Account Services. The petition form follows. Complete all sections of the form, and if necessary, provide documentation for your request. You may submit the completed form:

 

  • by Web: Fill out form below and click send email button.
  • by mail to Student and Departmental Account Services, P.O. Box 4079, Flagstaff, AZ 86011
  • in person to Student and Departmental Account Services, Gammage Building Lobby, Room 124
  • by fax to (928) 523-1126

Normally, notification of the action taken on the petition is done by e-mail, although phone or U.S. Mail are other options available if no e-mail account exists.

(Not for late registration fees or accounts receivable late fees.)

PETITIONS TAKE 4-6 WEEKS TO PROCESS

Late Payment Fees are not eligible for appeal once the relevant semester has concluded.
