Qualifications: No one will be refused service for financial reasons. Uninsured need income determination to determine fee for service. Available to entire community.

Fees: Insurance, Medicare and Medicaid accepted. Uninsured need proof of income and will be put on a sliding scale according to income.

8am-noon, 2pm-6pm, Monday-Friday
