Northland Hospice and Palliative Care
Services: An interdisciplinary team of professional staff and trained volunteers provide compassionate hospice and palliative care to patients with incurable and terminal illnesses in their own homes and provide support to their families and loved ones. Northland Hospice and Palliative Care is committed to helping patients live each day fully, with dignity and independence, and as free of pain as possible. Individual counseling and grief support groups are also available for all ages and circumstances.
Hospice staff travel to the home or site where the patient resides. Trained volunteers sometimes provide transportation to doctors appointments or other location on an as-needed basis.
Qualifications: Without regard to age, type of illness, race, gender, handicap, national origin, income level, or religious affiliation, we provide confidential service to all who meet this criteria:
- Patient is diagnose with a terminal illness
- Patient lives in Flagstaff or within a 50 mile radius
- A family member will serve as primary caregiver (or other suitable arrangements are made)
- Both the patient and the family desire hospice or palliative care in the home
Fees:
- Medicare and insurance coverage, private fees vary, sliding scale fees vary.
- Northland Hospice and Palliative Care provides services to all qualified applicants regardless of insurance coverage or ability to pay.
- No fee is charged for individual or group bereavement counseling and support; donations are accepted for this service.
Operating Hours:
8am-5pm, Monday-Friday.
Clinical staff on call 24/7.
