Blindness and Low Vision
Disability Verification Requirements
All information submitted to Northern Arizona University’s Disability Resources becomes part of a student’s “educational record” as defined by the Family Educational Rights and Privacy Act (FERPA).
To qualify for disability resources, students must submit a report from the professional(s) that diagnosed or evaluated the disability. The report must be typed on letterhead, dated and contain:
1. What methods/procedures were used to determine the diagnosis?
2. How does the condition's impact the individual in a university setting?
3. Does the condition substantially limit the person?
4. How should the condition(s) be treated?
5. How the disability affects major life activities and academic performance.
6. A list of professional recommendations for the student in a university setting with an explanation of how the recommendations will compensate for the disability.
BLINDNESS
Please provide copies of an assessment conducted by an M.D. or ophthalmologist. The assessment should demonstrate a certified loss of vision along with a written report explaining test results.
LOW VISION
Students with visual impairments should provide copies of a current assessment by an ophthalmologist. The assessment should demonstrate a certified loss (i.e., low vision) accompanied by a written report explaining test results and listing functional limitations in major life activities (e.g., reading, mobility) along with recommendations.
The information contained in the report will be used to determine eligibility. Appropriate accommodations will be based upon your disability and individual needs.
