Deafness or Hard of Hearing
Disability Verification Requirements
All information submitted to Northern Arizona University’s Disability Resources becomes part of a student’s “educational record” as defined by the Family Educational Rights and Privacy Act (FERPA).
To qualify for accommodations, students who are deaf or hard of hearing must provide a copy of a current assessment by a professional. The assessment must include an audiogram conducted by a certified audiologist and a written report explaining test results. The report should be typed on letterhead, signed, dated and must include the information listed below.
1. What methods/procedures were used to determine the diagnosis?
2. How does the condition's impact the individual in a university setting?
3. Does the condition substantially limit the person?
4. How should the condition(s) be treated?
5. How does the disability affect major life activities and academic performance?
6. A list of professional recommendations for the student in a university setting with an explanation of how the recommendations will compensate for the disability.
The information contained in the report will be used to determine eligibility. Appropriate accommodations will be based upon your disability and individual needs.
