How to use the forms on this page
Except to Self Identify, you must be eligible and registered for services with Disability Resources to receive these accomodations.
These forms will be available to either fill out online, download and fill out or both (you may also submit these forms, in person, at our office):
1. Online forms may be filled out and submitted through your browser.
2. Downloadable forms will be in .rtf format. The .rtf format will allow you to download, open, edit, and print/save in a text editor. You can either email, mail, fax or submit in person the .rtf format (if a signature is not required). If a signature is required you may email the unsigned copy and mail, fax or bring in the signed copy (we must have a signed copy on file).
3. Note: any form that requires a signature must be mailed, faxed or submitted in person.
