Students - Current and Prospective

How to apply:

To see if you are eligible for services, click here.

    Applying for Disability Resources services/accommodations is a three-part process:

  1. Submit your documentation to Disability Resources.

  2. Disability Resources staff will evaluate documentation and determine eligibility (evaluation process can take up to four weeks).

  3. Once you receive a letter of eligibility, call or come in to make an appointment with a Disability Resources coordinator to activate your services/accommodations.

IMPORTANT: It may take up to four weeks to review and/or process your documentation. Students are strongly encouraged to apply to Disability Resources for services/accommodations as early as possible prior to attending Northern Arizona University. Current students may apply for accommodation at any time but are encouraged to apply as soon as they become aware of a need for accommodation.

Documentation

To Self Identify and submit your documentation, follow the five steps below:

Step 1: Print, complete and submit a Self Identify form, along with any requested information in the remaining four procedures below, to Disability Resources.

Step 2: Refer to the Documentation Guidelines page.

Step 3: Contact the professional(s) that diagnosed your condition. Give them a copy of the requirements.

Step 4: Instruct the professional(s) to mail or fax the documentation to the contact information below. If you mail the documentation yourself, please keep your originals and send us photocopies only. You may also bring your documentation to the DR office.

Evaluation of Eligibility

Within four weeks of receiving your documentation, a letter will be mailed to you regarding your eligibility status with further instructions.

Considering the length of time it may take to evaluate the documentation, students are strongly encouraged to apply as early as possible prior to attending Northern Arizona University to ensure your accommodations are available the first day of the semester.

As a service to prospective Northern Arizona University students, Disability Resources will review disability related documentation regardless of a prospective student admission status.

All information submitted to Northern Arizona University’s Disability Resources becomes part of a student’s “educational record” as defined by the Family Educational Rights and Privacy Act (FERPA).

The information contained in the documentation will be used to determine eligibility.   Appropriate accommodations will be based upon your disability and individual needs.

Activate Services & Accommodations

To activate services/accommodations, call us at 928-523-8773 to schedule an appointment with a coordinator. Students must meet with a coordinator each semester to activating their services/accommodations.

The Disability Resources office is located in the Health & Learning Center, #25, Room 2050.

Extended Campus students (those not based on the Flagstaff campus) should call 928-523-8773 (voice) or 928-523-6906 (TTY) and ask for the Extended Campus coordinator for assistance in activating your services/accommodations.
