You must submit a Dependent Affidavit if: The EARP will be used for a dependent child with a different last name than the employee. The dependent applying for the EARP will be age 24 at any time during the calendar year in which the EARP is being applied. The dependent applying for the EARP is not listed as a beneficiary on the employee’s university benefits (i.e., health, dental). The EARP will be used for an affiliate's dependent child. Back to Top
