Student Health Insurance

Students will be auto enrolled for the insurance every semester.  Because eligibility is based upon current credit hours, you should check on your LOUIE account or call the Fronske Health Center to verify enrollment.

Who is Eligible?

  • Undergraduate students enrolled in at least 7 credit units. Seniors may enroll with less credits if they are in their last semester to achieve their final graduation requirements and they had the insurance coverage in the prior semester.

  • Graduate students in a degree or certificate program are eligible to enroll. Graduate, non-degree students must have applied to a degree program and be taking at least 6 transferable units, be in a certificate program or be a full-time student taking at least 9 units.

  • International students are automatically enrolled and premiums are posted to the students account at the time of enrollment.

  • Eligible dependents (spouse and children) of enrolled students may enroll in this program.

Enrollment deadline is 14 days after the first day of classes each semester

    To sign up for the Major Medical plan:
  1. Log on to LOUIE and click on "Purchase Student Health Plan/Insurance" on the self service page.
  2. Or, you may fill out an Enrollment Form and mail to Fronske Health Center.
  3. Or, call the Fronske Business Office at (928) 523-6343 to enroll over the phone

The deadline to enroll and pay any plan is 14 days after the first day of class. No enrollment will be accepted after this date unless a qualifying life circumstance exists, such as involuntary loss of other coverage through a job, parent, or spouse.

    Fronske Health Center Benefits
  • $10 office visit co-pay
  • $10 co-pay for laboratory and radiology services at Fronske
    In-Network Benefits
    (note, you must receive a referral from a Fronske provider for all off-campus facilities.)
  • $25 co-pay for mental health/chemical dependency (20 visits) - ($10 co-pay for NAU Counseling Center)
  • $35 co-pay charge for medical, women's health including maternity
  • $50 charge for urgent care center visits (for students, $25 when Fronske is closed)
  • $100 co-pay for emergency room visit (waived if admitted)
  • Hospitalizations (in-network) covered at 90% (maximum $1,500 out-of-pocket)
  • In-network outside laboratory and x-ray (not provided at Fronske) covered at 90% after the $250 deductible is met
  • Maximum out-of-pocket $1,500 in-network, $3,000 out-of-network for individuals, $2,000 maximum out-of-pocket in-network for families, $6,000 out-of-network maximum for families
    Other Benefits:
  • $300,000 lifetime maximum
  • Unlimited repatriation benefit
  • Unlimited medical evacuation benefit
