REGISTRATION

1. Print the registration form
   • Fall 2008 Dance Classes Registration Form
   • Science Festival Dance Workshop Registration Form
   • Music Lessons Registration Form
   • Suzuki String LessonsRegistration Form
   • Academy OrchestraRegistration Form
2. Complete the top portions of the form, both Student and Billing sections
3. Put a check mark in front of the instrument or class for which you are registering
4. Read and sign the back (second page). This section contains very important information; please read it carefully.
5. Fax the completed form to (928) 523-1133 or mail to Box 6040, Flagstaff, AZ 86011
6. Call the office at (928) 523-1889 with your credit card payment or mail your check to the above address.

Note: No one will be registered into a class or for lessons until 50% payment and a properly signed registration form has been received by the Academy office.
