REGISTRATION
- Print one or more of the following registration forms:
- 2010 Spring Academy Orchestra Registration Form
- 2010 Spring Ballet Registration Form
- 2010 Spring Tap and Jazz Workshops
- 2010 Spring Music Lessons
- 2010 Spring Suzuki
- 2010 Spring Fiddle (COMING SOON!)
- Complete the Student and Billing sections at the top of each form.
- Select the instrument or class for which you are registering
- Read and sign the second page (back). This section contains very important information; please read it carefully.
- Fax the completed form to (928) 523-1133 or mail to Box 6040, Flagstaff, AZ 86011.
- Call the Academy office at (928) 523-1889 with your credit card payment or mail your check to the above address.
Note: No one will be registered in a class or for lessons until 50% payment and a properly signed registration form have been received in the Academy office.
